New Employee Paperwork Instructions

Newly hired student employees must print out and complete the following forms. All forms must be completed using BLACK ink. If mistakes are made do not use white out or correction tape, start a new form. Read each form carefully and make sure you sign each form as required. All completed forms must be returned within one week of hiring. You may not be permitted to start working until all paper work is completed. Completed forms must be returned to PE043.

• I-9 Documentation Please bring ORIGINAL identification as indicated on this sheet. Most employees provide their Driver's License AND Social Security Card or Birth Certificate.
